Introduction

Fastaff, LLC and U.S. Nursing Corporation (collectively, “Fastaff or We”) respect your privacy and are committed to protecting it through our compliance with this policy.

This policy describes the types of information we may collect from you or that you may provide when you visit www.Fastaff.com, www.usnursing.com, nursejob.com, or fastaff.erecruitcloud.com (collectively, the “Websites”) and our practices for collecting, using, maintaining, protecting, and disclosing that information.

This policy applies to information we collect:

  1. On the Websites.
  2. In email, text, and other electronic messages between you and the Websites.
  3. Through mobile and desktop applications you download from the Websites, which provide dedicated non-browser-based interaction between you and the Websites.
  4. When you interact with our advertising and applications on third-party websites and services, if those applications or advertising include links to this policy.

It does not apply to information collected by:

  1. us offline or through any other means (except when we enter such information into our electronic systems), including on any other website operated by Fastaff or any third party (including our affiliates and subsidiaries) unless otherwise expressly specified on such website; or
  2. any third party (including our affiliates and subsidiaries), including through any application or content (including advertising) that may link to or be accessible from or on the Websites.

Please read this policy carefully to understand our policies and practices regarding your information and how we will treat it. If you do not agree with our policies and practices, your choice is not to use our Websites. By accessing or using the Websites, you agree to this privacy policy. This policy may change from time to time (see Changes to Our Privacy Policy). Your continued use of the Websites after we make changes is deemed to be acceptance of those changes, so please check the policy periodically for updates.

Children Under the Age of 13

Our Websites are not intended for children under 13 years of age. No one under age 13 may provide any information to or on the Websites. We do not knowingly collect personal information from children under 13. If you are under 13, do not use or provide any information on any of the Websites or on or through any of its features/register on any of the Websites or provide any information about yourself to us, including your name, address, telephone number, email address, or any screen name or user name you may use. Information We Collect About You and How We Collect It

We collect several types of information from and about users of our Websites, including information:

  1. by which you may be personally identified, such as name, birth date, postal address, e-mail address, telephone number, social security number or ANY OTHER INFORMATION THE WEBSITES COLLECT THAT IS DEFINED AS PERSONAL OR PERSONALLY IDENTIFIABLE INFORMATION UNDER AN APPLICABLE LAW (”personal information”);
  2. that is about you but individually does not identify you; and/or
  3. about your internet connection, the equipment you use to access our Websites and usage details.

We collect this information:

  1. Directly from you when you provide it to us.
  2. Automatically as you navigate through the sites. Information collected automatically may include usage details, IP addresses, and information collected through cookies and other tracking technologies.

Information You Provide to Us.

The information we collect on or through our Websites may include:

  1. Information that you provide by filling in forms on our Websites. This includes information provided at the time you register to use the services of Fastaff if you represent a hospital; apply for employment with Fastaff; register to use the Nurse Portal login, the Fastaff Mobile Application, or other services made available on the Websites. We may also ask you for information when you report a problem with our Websites.
  2. Records and copies of your correspondence (including email addresses), if you contact us.
  3. Details of transactions you carry out through our Websites, including without limitation, fulfillment of staffing requests and nurse job applications.
  4. Your search queries on the Websites.
  5. Information and documentation you choose to upload using the Fastaff document repository available to nurses through the Websites (the “Repository”).

You also may provide information to be published or displayed (hereinafter, “posted”) on public areas of the Websites, or transmitted to other users of the Websites or third parties (collectively, “User Contributions”). Your User Contributions are posted on and transmitted to others at your own risk. Although we limit access to certain pages, please be aware that no security measures are perfect or impenetrable. Additionally, we cannot control the actions of other users of the Websites with whom you may choose to share your User Contributions. We cannot guarantee that third party vendors engaged by us who have access to User Contributions in connection with the services they perform will not provide access to User Contributions without authorization. We are not responsible for the actions or omissions of third party vendors. Therefore, we cannot and do not guarantee that your User Contributions will not be viewed by unauthorized persons.

Information We Collect Through Automatic Data Collection Technologies.

As you navigate through and interact with the Websites, we may use automatic data collection technologies to collect certain information about your equipment, browsing actions, and patterns, including:

  1. Details of your visits to the Websites, including traffic data, location data, logs, and other communication data and the resources that you access and use on the Websites.
  2. Information about your computer, mobile device, and internet connection, including your IP address, operating system, browser type and other browser information.

We also may use these technologies to collect information about your online activities over time and across third-party websites or other online services.

The information we collect automatically is statistical data and does not include personal information. It helps us to improve our Websites and to deliver a better and more personalized service, including by enabling us to:

Pages of the Websites and our e-mails may contain small electronic files known as web beacons (also referred to as clear gifs, pixel tags, and single-pixel gifs) that permit Fastaff, for example, to count users who have visited those pages or opened an email and for other related website statistics (for example, recording the popularity of certain website content and verifying system and server integrity).

We do not collect personal information automatically, but we may tie this information to personal information about you that we collect from other sources or you provide to us.

Third-Party Use of Cookies and Other Tracking Technologies.

Some content or applications, including advertisements, on the Websites are served by third-parties, including advertisers, ad networks and servers, content providers, and application providers. These third parties may use cookies alone or in conjunction with web beacons or other tracking technologies to collect information about you when you use our website.

We do not control these third parties’ tracking technologies or how they may be used. If you have any questions about an advertisement or other targeted content, you should contact the responsible provider directly. 

How We Use Your Information

We use information that we collect about you or that you provide to us, including any personal information:

  1. To present our Websites and their contents to you.
  2. To provide you with information, products, or services that you request from us.
  3. To fulfill staffing requests or assignments that you make and to fulfill any other purpose for which you provide it.
  4. To provide you with notices about your relationship with Fastaff.
  5. To notify you about changes to our Websites or any products or services we offer or provide though it.
  6. To allow you to participate in interactive features on our Websites.
  7. To help you with job placements or assignments
  8. In any other way we may describe when you provide the information.

We may disclose aggregated information about our users and information that does not identify any individual, without restriction.

We may disclose personal information that we collect or you provide as described in this privacy policy:

To a buyer or other successor in the event of a merger, divestiture, restructuring, reorganization, dissolution, or other sale or transfer of some or all of Fastaff’s or U.S. Nursing’s assets, whether as a going concern or as part of bankruptcy, liquidation, or similar proceeding, in which personal information held by Fastaff or U.S. Nursing about our Websites’ users is among the assets transferred.
  4. To fulfill the purpose for which you provide it.
  5. For any other purpose disclosed by us when you provide the information.

We strive to provide you with choices regarding the personal information you provide to us. We have created mechanisms to provide you with the following control over your information:

  1. Tracking Technologies and Advertising. You can set your browser to refuse all or some browser cookies, or to alert you when cookies are being sent.

Data Security

We have implemented measures designed to secure your personal information and information you upload to the Repository from accidental loss and from unauthorized access, use, alteration, and disclosure. No security measures are perfect or impenetrable. The safety and security of your information also depends on you. Where we have given you (or where you have chosen) a password for access to a part of one of our Websites, you are responsible for keeping this password confidential. We ask you not to share your password with anyone. Unfortunately, the transmission of information via the internet is not completely secure. It is our policy to post any changes we make to our privacy policy on this page with a notice that the privacy policy has been updated on the Websites’ home pages. If we make material changes to how we treat our users’ personal information, we will notify you by email to the primary email address specified in your account and through a notice on the effected Website’s home pages. The date the privacy policy was last revised is identified in the privacy policy.
